In the paper-based exam, candidates write their answers on paper for the Listening, Reading, and Writing sections, whereas in the computer-based version, these sections are completed on a computer. The Speaking test remains the same for both, conducted face-to-face with an examiner. From my perspective, choosing between the two often depends on your comfort and proficiency with typing and navigating a computer.
